Trinity at Home

Trinity at Home provides an extra level of support to give patients, families and carers the practical help and reassurance they need to continue care at home when a person’s needs become more complex and demanding – during the last days of life, for example, or if a person’s illness suddenly worsens.

This service recognises that caring for someone we love when we know they may be nearing the end of life is difficult and emotionally draining. Family carers, no matter how willing and determined, can become exhausted and lose confidence when symptoms may be unpredictable and changing fast. Feedback from families tells us that the right help at the right time makes a huge difference.

The nature of the support provided depends on needs – for example, a patient may need a nurse to be with them through the night or a carer or family member may need a short break. 

Importantly Trinity at Home helps to avoid distress by reducing unnecessary emergency admissions to hospital, enabling individuals to die, peacefully at home with their friends and families around them, should that be their wish. 

Our Trinity at Home service is supplementary to the care at home provided by our clinical nurse specialists.
