How home care works

Trinity’s specialist nurses visit regularly to help by:

• discussing how you are feeling and assisting with pain relief and symptom control
• advising your GP your about changes to your medication
• listening to any concerns you have, on any topic, and helping you to access other services that may benefit you
• ensuring everyone involved with your care understands your needs and wishes
• working with you and your family to plan for the future

Trinity’s nurses make home visits every day between 8.30am and 5pm. The frequency of visits depends on individual needs. They also provide 24-7 telephone support.

Our nurses can also put you in touch with Trinity’s Clapham-based outpatient services, which may benefit you and your family. These include clinics, counselling, therapies, treatments, creative activities and support groups. Our nurses can also tell you in touch with our Inpatient Centre staff if you would like to know more about inpatient services.

Working with all your care providers

Trinity’s nurses play an important role in helping to coordinate all the services you need to continue care at home. Your GP, district nurse, hospital consultants and local social service department will continue to be responsible for your care. Trinity’s nurses will stay in touch with and advise them as needed. Your GP will continue to prescribe your medicine.
