Who are Trinity’s home care services for?

Trinity at Home is for people who are living with an advanced illness and prefer to be cared for at home. We support people living in the boroughs of Wandsworth, Sutton, Merton, Richmond and Twickenham, Kingston, Hammersmith and Fulham, Kensington and Chelsea, and Westminster.

Referrals

You can talk to us directly about your needs by calling our Trinity at Home administrator on 020 7787 1062 or our main switchboard on 020 7787 1000. Referrals can also be made through your GP, district nurse or hospital consultant.

If you are already using Trinity’s inpatient or day therapy services, you can talk to any member of our nursing team.
