Specialist nurse support at home
Trinity’s highly skilled clinical nurse specialists visit patients in the comfort and security of their own homes. They provide an extra level of help and support that is complementary to the care an individual may already be receiving from their GP, district nurse or hospital consultant.
Our skilled and experienced nurses understand that living with a life-threatening illness may raise many questions and worries and they take the time to get to know their patients, family members and carers. They can help by:
- working closely with their patients to develop a personal care plan tailored to individual needs
- providing specialist information, advice and support and help with pain management and other troublesome symptoms
- coordinating all the different services that are involved in a person’s care at home including GPs, district nurses, hospitals and social services
Each of our patients has a named nurse who is the person they see the most. In some areas, two nurses work together.
Trinity’s specialist nurse support is available to patients and carers seven days a week in working hours. Telephone advice and support is available out of hours.
