Education and support for health and social care professionals

As a community hub for specialist end of life care, Trinity is a resource for other local health and social care professionals. Through education and support for other care providers, we help to raise standards and build confidence and skills so that many more people experience good care at the end of their lives.

Supporting GPs, district nurses, care home staff and others

In line with the Government’s National End of Life Care Strategy, Trinity provides training and support to many professionals to help them gain skills and confidence in delivering end of life care.

GPs

Trinity works with GPs to help them ensure that the end of life care needs of their patients are met. This involves working in partnership to raise awareness of the needs of patients as well as building confidence and skills in helping patients to discuss their preferences and advance care plans.

Trinity also offers short workshops for GPs, designed around their busy schedules, and training days.

District nurses

Trinity works closely with district nurses in our area to strengthen our relationships and explore what really good end of life care looks like and how we can make it happen together. We offer comprehensive training to help district nurses gain an understanding of and confidence in caring for people at the end of life. This involves a mixture of training days at Trinity and workplace learning sessions.

Other training and support

Care homes

Trinity works with care homes in our catchment area to provide support and advice and help staff to develop the skills they need to enable their residents to live in their care home until they die and avoid unnecessary and often distressing emergency admissions to hospital.

Care for people with dementia

With local NHS funding, Trinity has a specialist dementia care nurse working with GPs and other primary healthcare professionals in Wandsworth. The aim is to ensure that people with dementia have access to the same palliative care service as people with other life-limiting illnesses such as cancer.

Communication skills training

Trinity offers communication skills training in end of life care to health and social care professionals in all roles. The aim is to develop these skills so that patients and families are helped to talk about the issues that matter to them, which in turn helps to ensure that they receive appropriate support that recognises their preferences and minimises distress.

Tailor-made training

Trinity can also be commissioned to provide bespoke training workshops to a range of professionals who may wish to improve their services, skills and abilities to address the needs that arise at the end of a person’s life.
