Information for patients, families and carers.

 

Trinity cares for people with many different types of illnesses such as cancer, heart, lung and kidney diseases and also neurological disorders such as motor neurone disease and end stage dementia. It’s the needs of our patients that matter to us rather than the diagnosis. 

Most referrals to Trinity are made by the health care professional involved in the patient’s care such as a GP or hospital nurse. Patients and families can also 'self-refer' but do need to provide information from their GP or hospital. Please feel free to call our referral and admissions nurse if you would like to chat about the possibility of you or someone close to you being cared for by Trinity. 

Trinity provides care at home and in our Inpatient Centre as well as day services and family support – you can read all about all these services in this section.

We also have a small number of beds for respite care to give families friends or carers a short break, usually for a week, from the physical and emotional demands of caring for someone who is very ill.

Our catchment area covers:

  • Hammersmith & Fulham
  • Kensington & Chelsea
  • Lambeth
  • Richmond & Twickenham
  • Southwark
  • Wandsworth
  • Westminster

We sometimes care for people from other areas if, for example, close family and friends live in our catchment area.
