Our services
Trinity Hospice has four main services:
- Community Palliative Care (Specialist nurse support at home)
- Trinity at Home
- Mulberry Place day services
- Inpatient Centre
Respite care
At our capacity of 28 beds, we can accept a limited number of patients for respite when families and friends need a break. We can offer these breaks for one week, ideally booked in advance. The patient’s symptoms must be controlled and their condition stable.
Our catchment area
Trinity covers the following areas:
- Hammersmith & Fulham
- Kensington & Chelsea
- Lambeth
- Richmond & Twickenham
- Southwark
- Wandsworth
- Westminster
We sometimes care for people from other areas if, for example, close family and friends live in our catchment area.
Referral criteria
- The patient has a progressive disease that cannot be cured, or the patient has refused treatment if competent to do so. Any disease is appropriate for our services including non-cancer conditions.
- Complex problems including symptom control, psychological, social or spiritual issues that are important to the patient
- The patient is entering the terminal phase/imminently dying
- Assessments suggest that the patient’s care would be best dealt with through palliative care in the community, through our day services or as an inpatient in a hospice setting
- Where the patient is competent, they must have consented to their referral.

How to refer a patient
Referrals are accepted from all health and social care professionals. When the patient is already an inpatient elsewhere we ask that wherever possible they have a palliative assessment by, for example, the hospital palliative care team.
Our Referral and Admissions Nurse works 08:30-16:30 Monday to Friday. Please get in touch if you would like to discuss an admission or if you have any questions.
Referrals are made by completing our referral form and returning it to us with the appropriate supporting documentation* by fax or by post. If the referral is urgent (within 48 hours) please phone directly for advice.
* Supporting documentation Any recent clinic letters, discharge summaries, investigations or current medications are helpful.
For community referrals, we also request that they are supported by a district nurse referral as we need to work with them to effectively meet the needs of community-based patients.
